The father of a child whose name is not on the birth certificate may obtain a form called Affidavit of Parentage from the Probate Court office in order to acknowledge that he is the father of the child.  The form has instructions for completion and filing the form in Lansing, MI.

This procedure is meant for use by couples who are not married.  If the child was conceived or born to a married woman, her husband is presumed to be the father unless a court determines otherwise.  In this circumstance, legal advise is recommended.
